Scope creep is the gradual expansion of a project beyond what was agreed, without a matching increase in time or money. The word "gradual" is doing the work — nobody experiences it as a single moment. It arrives as a sequence of small, individually reasonable requests, each easy to say yes to.
That is why it is so hard to push back on. Refusing any single request looks petty. It is only when you add them up that the picture changes, and by then you have established a pattern of saying yes.
It is also a documents problem far more than a personality problem. Four pieces of writing prevent most of it, and all four go in the scope of work you send before the project begins.

What causes scope creep?
Almost always a scope document that lists what is included but not what is excluded. When nothing states that something is outside the project, both sides fill the gap with their own assumption, and those assumptions differ. Undefined revision rounds and having no single named approver are the next two causes.
How do I tell a client something is out of scope?
Lead with agreement rather than refusal. 'Happy to take that on — it sits outside the original scope, so it would be around [amount] and add [time] to the timeline. Want me to go ahead?' You are not saying no; you are saying yes with a price.
When should I raise it?
The first time it happens. Three yeses establish a precedent, so raising it on the fourth request genuinely does look like a rule you have just invented. Raising it on the first is unremarkable.
